HOMECARE is the care given to a patient in the house (if required) after being discharged from the hospital. It can be acute or chronic illnesses. The training will include treating and caring for patients with or without a carer. Multi-Disciplinary Team concept in collaboration with HOMEGP
HOMEGP is an online platform that connects healthcare seekers to house-call healthcare practitioners. We at MHAS are working hand-in-hand to build a team of advocates with the Multi-Disciplinary Team concept – where members may have separate but interrelated roles and advise within their disciplinary boundaries.
It starts with the patient being assessed individually by several professionals (such as practitioners, nursing, social workers, psychiatrists, medical, etc), and then coming together as a whole to discuss their individual assessments and develop a joint service plan for the patient.
If you are keen to join our team in this approach, please email your CV (max 2 to 3 pages), highlighting your last five years of experience, to info@healthyageing.org.
Please indicate in the email Ref/Subject/Title: CV for the Roadshow series.
